The ATTAIN-MAINTAIN trial studied whether oral orforglipron could help people keep weight off after they had already lost weight on injectable treatment.

The study included 376 adults who had previously completed 72 weeks of injectable treatment.

One group had previously used Mounjaro, and the other had previously used Wegovy injections.

Participants were then switched to either once-daily oral orforglipron or placebo for 52 weeks.

What percentage of people maintained at least 80% of their previous weight loss?

The study also looked at how many people maintained at least 80% of the weight they had already lost.

This means people taking orforglipron were much more likely to maintain the majority of their previous weight loss than those who stopped active treatment.

What does this mean in real terms?

If someone lost 20kg on injectable treatment, maintaining:

  • 75% would mean keeping off around 15kg
  • 50% would mean keeping off around 10kg
  • 38% would mean keeping off around 7.6kg

So, in practical terms, orforglipron may help reduce the amount of weight regained after stopping injectable treatment.

It does not mean everyone will maintain all of their weight loss, but the trial suggests that continuing with an oral GLP-1 can help preserve more weight loss than stopping active treatment completely.

Is orforglipron better than stopping treatment?

Based on ATTAIN-MAINTAIN, yes. People who switched to orforglipron maintained significantly more of their previous weight loss than people who switched to placebo.

This supports the idea that weight loss treatment may need a long-term maintenance phase, similar to how blood pressure or cholesterol treatment is often continued long term.

The study does not prove that orforglipron is better than staying on injectable treatment. It compared switching to orforglipron against switching to placebo after previous injectable treatment.

GLP-1 tablets for maintaining weight loss

GLP-1 tablets could become useful for maintenance because they may be easier to take long term than injections.

For some people, the goal may not be to keep losing large amounts of weight. Instead, the goal may be to maintain the weight already lost and prevent regain.

What dose of orforglipron was used?

The ATTAIN-MAINTAIN study used the investigational orforglipron capsule formulation, up to 36mg or the maximum tolerated dose.

The paper states that these capsule doses correspond to tablet doses, with the 36mg capsule broadly corresponding to the 17.2mg tablet dose.

A simple way to explain this is:

Earlier orforglipron trials used capsules, while the approved product uses tablets with different dose labels. The highest trial dose, 36mg, is considered broadly equivalent to the highest tablet dose, 17.2mg, because both provide similar drug exposure in the body.

Are there similar maintenance studies for Wegovy tablets?

At present, there does not appear to be a dedicated maintenance study showing that Wegovy tablets help people keep weight off after switching from injectable Wegovy or Mounjaro.

The OASIS trials show that oral semaglutide can produce meaningful weight loss while treatment continues, but they were not designed specifically as post-injection maintenance studies.

This makes orforglipron’s ATTAIN-MAINTAIN data important, because it directly studied oral GLP-1 treatment as a maintenance option after injectable treatment.

How to maintain weight loss after injectable GLP-1 treatment

Medication can help, but long-term weight maintenance also depends on habits and follow-up support.
