Which Tadalafil Dose Should I Choose?

Choosing the right Tadalafil dosage depends on how often you plan to have sex, how quickly you want the treatment to work, and how your body responds to the medicine.

Tadalafil 2.5mg or 5mg tablets (daily use)

If you prefer spontaneity or want to avoid planning around sexual activity, a once-daily low dose is usually the best option.

Instructions: Take one tablet daily

2.5mg: The lowest strength, ideal for men who are sensitive to side effects or want a gentler introduction to treatment. It builds up gradually in your system and offers continuous coverage after a few days.

5mg: The most commonly prescribed daily dose. It’s strong enough to provide reliable results while still being suitable for long-term use. Many men also find it helps with urinary symptoms caused by benign prostatic hyperplasia (BPH).

Best for: regular sexual activity, couples who prefer spontaneity, or those with ED and urinary symptoms.

Tadalafil 10mg or 20mg tablets (on-demand use)

If you only want to take Tadalafil when you need it, an on-demand dose might be more suitable.

Instructions: Take one tablet 30-60 minutes before sexual activity.

10mg: Usually the recommended starting dose for occasional use. Take it 30–60 minutes before sexual activity.

20mg: A higher-strength option if 10 mg isn’t effective enough. It offers a more powerful response but also carries a higher chance of side effects.

Best for: occasional sexual activity or when you want a stronger effect.

Like all prescription medicines, Tadalafil can cause side effects. Not everyone experiences them, and most are usually mild and short-lived. There are no known long-term side effects associated with Tadalafil.

Common Tadalafil side effects (affecting up to 10% of users)

These are the most frequently reported side effects and often improve as your body gets used to the medication:

• Headache
• Muscle pain
• Flushing (warm, red skin)
• Nasal congestion or a blocked nose
• Indigestion (dyspepsia)

While rare, some side effects require immediate medical attention. Seek urgent medical attention if you experience:

• A painful erection or one lasting more than 4 hours (priapism)
• Sudden loss or changes to vision or hearing
• Signs of a severe allergic reaction, including difficulty breathing, severe rash, or swelling of the lips, eyes, mouth, or throat

You should speak to your doctor about taking Tadalafil if you have:

• Recently had a heart attack or stroke (within the last 3 months)
• An eye condition called optic neuropathy
• Low blood pressure
• Heart failure
• Been advised to avoid exercise or sexual activity
• Abnormal heart rhythm, angina or high blood pressure that is not controlled
• Deformation of the penis
• A higher chance of a prolonged erection (e.g. if you have sickle-cell disease, multiple myeloma or leukaemia)
• Stomach ulcers

Tadalafil is generally safe to take with most medications. You should speak to your doctor before taking Tadalafil if you are taking:

• Alpha blockers such as alfuzosin, doxazosin or tamsulosin
• Medication to treat blood pressure
• Any heart medication
• Medication to treat epilepsy
• Antifungal treatment
• Crizotinib, enzalutamide, imatinib, nilotinib, idelalisib prescribed to treat some cancers
• HIV medication
• Erythromycin, clarithromycin, or rifampicin (antibiotics)
• Netupitant or aprepitant prescribed to treat sickness
• Nitrates
• St John’s Wort
• Sapropterin

If you are taking any of these medications, you may still be able to take Tadalafil, but your doctor may wish to monitor you, or you may need to leave a gap before taking a tablet.
