What are GLP-1 tablets?

How do GLP-1 tablets work?

How effective are GLP-1 tablets?

Clinical trials have shown that oral semaglutide can support weight loss in adults with overweight or obesity.

In the OASIS 4 phase 3 trial, oral semaglutide 25mg was studied in adults with overweight or obesity without type 2 diabetes. After 64 weeks, average body weight reduced by 13.6% with oral semaglutide 25mg, compared with 2.2% with placebo.

Among people who adhered to treatment, average weight loss was 16.6% with oral semaglutide 25 mg, compared with 2.7% with placebo.

Individual results vary. GLP-1 tablets are not a quick fix and work best as part of a structured weight-management plan.

How to take GLP-1 tablets

The instructions depend on the type of GLP-1 tablet. In the UK, Wegovy tablets are licensed for weight management, while orforglipron is still being studied in clinical trials and is not currently licensed.

Wegovy tablets are taken once daily on an empty stomach. They should be swallowed whole with a small amount of plain water after fasting for at least 8 hours. You should then wait at least 30 minutes before eating, drinking or taking other oral medicines.

Orforglipron is an investigational oral GLP-1 medicine. In clinical trials, it was studied as a once-daily tablet without the same food and water restrictions used for oral semaglutide. These were trial instructions only, not approved UK patient instructions.

Who can and cannot take GLP-1 tablets

You may be able to take GLP-1 tablets if… GLP-1 tablets may not be suitable if…
You are an adult with obesity, usually BMI 30 or above. You are pregnant, trying to conceive, or breastfeeding. Semaglutide should be stopped at least 2 months before a planned pregnancy.
You are overweight, usually BMI 27 or above, and have a weight-related health condition such as high blood pressure, high cholesterol, pre-diabetes, type 2 diabetes, sleep apnoea or heart disease. You are under 18, unless specifically assessed by a specialist.
You want a prescription weight-loss treatment that is taken by mouth instead of injected. You only want to lose a small amount of weight for cosmetic reasons. GLP-1 medicines should not be used for aesthetic weight loss.
You are willing to use treatment alongside a reduced-calorie diet and increased physical activity. You have previously had a serious allergic reaction to semaglutide or any ingredient in the tablet.
You can take the tablet correctly: on waking, after fasting, with up to 120ml water, then wait at least 30 minutes before food, drink or other medicines. You have a history of pancreatitis, or symptoms that could suggest pancreatitis, unless a prescriber decides treatment is appropriate.
Your medical history and current medicines have been reviewed by a prescriber. You have severe gastroparesis or significant problems with delayed stomach emptying.
You understand the possible side effects, including nausea, vomiting, diarrhoea, constipation, reflux and abdominal discomfort. You have type 1 diabetes or diabetic ketoacidosis.
You are not taking another GLP-1 medicine at the same time, such as Wegovy injections, Ozempic, Rybelsus or Mounjaro. You are already using another GLP-1 or GIP/GLP-1 medicine, unless switching under clinical guidance.
You can commit to ongoing weight, side effect and safety monitoring. You have severe kidney disease, severe liver disease, unstable diabetic eye disease, or complex medical problems that require specialist assessment.

What is the best GLP-1 tablet?

In the UK, Wegovy tablets are currently the main GLP-1 tablet licensed for weight management, so there is no direct licensed oral GLP-1 weight-loss tablet to compare it with. The best option depends on your BMI, medical history, side effects, preferences and prescriber assessment.

Wegovy tablets contain semaglutide and are taken once daily. Other oral GLP-1 medicines such as orforglipron are still being studied and should not be treated as available UK weight-loss treatments unless they become licensed.

Side effects of GLP-1 tablets

The most common side effects of GLP-1 tablets are usually:

  • nausea
  • vomiting
  • diarrhoea
  • constipation
  • indigestion
  • abdominal pain

These are usually most noticeable when starting treatment or increasing the dose.

In clinical trials, around 8% of people taking orforglipron stopped treatment due to side effects, compared with 6.8% for Wegovy tablets. In both cases, stomach-related side effects such as nausea, vomiting and diarrhoea were the main reason people stopped treatment.

Are GLP-1 tablets safe?

For the right patient, prescribed through the correct route, GLP-1 tablets can be appropriate and effective treatments. However, they are still prescription medicines and are not suitable for everyone. Both Wegovy tablets and orforglipron carry warnings around important risks and require clinical screening before prescribing.

As with other GLP-1 medicines, they are generally not suitable for people with a personal or family history of medullary thyroid carcinoma or MEN2, and caution is needed in people with certain gastrointestinal issues, pancreatitis concerns, gallbladder disease, or other relevant medical conditions.
